What you are told before any treatment, how consent is recorded, and your right to change your mind at any stage.
No treatment goes ahead until you have been given enough information to make a real decision. That means what the treatment involves, what it can and cannot achieve, the likely outcome, the risks and side effects, the alternatives, and what happens if you do nothing.
Consent is a conversation, not a signature. The form records the discussion, it does not replace it.
Treatments are only carried out on adults aged 18 or over. Photographic identification may be requested.
Consent is only valid where you have capacity to give it, are acting voluntarily, and have been given the information above.
You are never asked to decide on the spot. For injectable treatments in particular you are welcome to take the information away and book separately once you have thought about it.
Where a treatment is significant or irreversible, a period of reflection between consultation and treatment is encouraged.
You can withdraw consent at any point, including after a treatment has been booked, a deposit paid, or the appointment has begun. Deposit terms are set out in the cancellation policy, but nobody is ever treated against their wishes.
